SUMMARY:

Provide administrative, clinical, and psychosocial support to optimize outcomes for patients and their families as a key member of the interdisciplinary palliative care team.  Work within multi-disciplinary teams to optimize outcomes for patients and their families.

ESSENTIAL FUNCTIONS:

Attend interdisciplinary team meetings to review active cases and assess needs.

Coordinate team efforts to ensure smooth discharge planning, clear follow-up, and continuity of care after discharge.

Coordinate patient/family meetings in clarifying goals of care, addressing advance care planning needs, code status, and psychosocial considerations.

Assist/intervene with difficult decision making and coping strategies.

Attend team meetings addressing the operational and administrative functions of the palliative care service.

Assist in development of policy and procedures, objective tools, which direct palliative care processes.

Coordinate bereavement services for patients and their families with TMC Hospice. 

Advance bereavement services by attending monthly bereavement committee meetings and promoting consistent education and resources to hospital staff.

Assist Medical Director and interdisciplinary team with education and outreach services.

Participate in quality improvement initiatives that traverse health care settings.

Serve as a liaison to ensure care coordination, appropriate resource allocation, and patient satisfaction throughout the continuum of care.

Adhere to TMCH organizational and department-specific safety, confidentiality, values, policies and standards.

Perform related duties as assigned.

MINIMUM QUALIFICATIONS

EDUCATION:  Master in Social Work or equivalent.

EXPERIENCE:   Minimum of two (2) years of acute care/hospital based practice.  Background in mental health or counseling is desirable.  Prior experience working in palliative care or hospice preferred.

LICENSURE OR CERTIFICATION:  Must have or be able to obtain Arizona State licensure as a LCSW at the time of hire.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of computer application software such as Microsoft Office products.

  • Knowledge of relevant laws, regulations, and hospital policies.

  • Clear, concise verbal and written communication skills. Works collaboratively.

  • Skill in evaluating cases and determining appropriate discharge plans.

  • Knowledge of psychosocial factor influencing behavior, counseling concepts, and discharge planning.

  • Knowledge of interventions, skills and techniques for complex psychosocial situations.

  • Ability to present relevant topics to individuals and small or large groups..

  • Ability to provide therapeutic techniques to provide problem resolution.

  • Ability to define problems, collect data, establish facts, and draw valid conclusions.

  • Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
