Social Worker - Palliative Care
Conduct face to face sessions for patients with serious illnesses and their family members in order to complete thorough assessments, provide supportive counseling and/or crisis interventions. Support interdisciplinary team members in an effort to maximize each patient’s potential for healthy coping.
Conduct psycho social assessments of identified patients and families to ensure coordinated psycho social support with other disciplines or community resources. .
Interprets social, emotional and family problems and relays them to palliative care providers /other team members via rounds and documents as often as indicated.
Provide emotional support to patients and families and /or groups of patients that may exhibit feelings of anxiety and stress brought on by their illness and hospitalization. .
Coordinate/ attend patient/family meetings to clarify goals of care, advance care planning needs, and code status. Follow up with families to provide support, answer questions or identify concerns throughout the stay.
Maintain acceptable working knowledge of resources needed to provide patients with continuity of care before and after discharge.
Develop and coordinate the implementation of a grief /loss support group for families of patients that do not meet criteria for hospice support.
Co –Chair monthly bereavement committee meetings which promote consistent education and resources to hospital staff.
Participate in palliative care quality improvement initiatives, education and research activities.
Implement a 30 day follow up call process to discharged palliative care patients to prevent /reduce re admission to the hospital.
Adhere to TMCH organizational and department-specific safety, confidentiality, values, policies and standards.
Perform related duties as assigned.
EDUCATION: Master in Social Work or equivalent required.
EXPERIENCE: Minimum of two (3) years of recent acute care/hospital based practice required. Background in mental health or counseling preferred. Prior experience working in palliative care or hospice preferred.
LICENSURE OR CERTIFICATION: Must have or be able to obtain Arizona State licensure as a LCSW at the time of hire.
KNOWLEDGE, SKILLS AND ABILITIES:
Working knowledge of computer application software such as Excel, PowerPoint and Visio.
Knowledge of relevant laws, regulations, and hospital policies.
Current knowledge of legal, regulatory and accrediting requirements and standards related to the practice of social work.
Ability to organize and establish priorities.
Ability to establish effective inter personal relationshipswith all levels of personnel , medical staff, patients/families and external resources ,
Clear, concise verbal and written communication skills.
Ability to work collaboratively with internal /external customers, colleagues and team members.
Skill in evaluating cases, determining patient needs and coordinating with the appropriate interdisciplinary team members.
Knowledge of psychosocial factor influencing behavior, interventions, techniques to be utilized for complex situations.
Ability to present relevant topics to individuals and small or large groups.
Ability to define problems, collect data, establish facts, and draw valid conclusions.
Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
