Our Geropsychiatric Center is a 16 bed Specialty Hospital licensed to provide short- term, inpatient mental health to older adults. A full range of services are offered for patients with major psychiatric diagnoses. The patient population served is adults 65 and over. Services may be provided to individuals 55-64 if they have issues of aging (medical and/or cognitive) that would preclude them from admission to an adult behavioral health unit. All patients will be medically stable prior to admission. High-quality treatment is provided by a group of highly trained health professionals in a caring and compassionate environment. We specialize in treating acute emotional and behavioral disorders related to aging, such as depression, dementia and adjustment difficulties. An older adult may be in need of treatment when experiencing a severely disordered behavior that is out of the norm with their typical behavior.  Services provided will include psychiatric evaluation and treatment; physician history and physical and ongoing medical oversight; individual, group and family therapy; recreational therapy and leisure activities/skills, occupational therapy, treatment planning, medication management, 24 hour nursing care, therapeutic milieu and discharge planning.

SUMMARY:

Coordinates plan of care across disciplines with expertise in social work, crisis intervention, psychosocial concerns and discharge planning. Provides individual, group and family therapy and maintains the therapeutic milieu. Works within multi-disciplinary teams to optimize outcomes for patients and their representatives. Ensures supervision of social work team.

ESSENTIAL FUNCTIONS:

Evaluates psychosocial factors influencing health status, integrating these into the plan.

Collaborates with primary nurse/physician and other members of the team, creating consensus around issues of discharge planning.

Documents in a clear, complete, concise, and organized manner.

Assesses referrals and identifies patients who qualify for inpatient treatment

Assesses, intervenes and provides resource management to patients and families.

Schedules and facilitates weekly care conferences

Develops treatment plan and discusses proposed course of treatment with multidisciplinary team

Follows through to ensure patient/family/staff concerns are appropriately resolved in a timely manner.

Evaluates social work processes; initiates positive change when indicated to ensure efficiency and maximize for the greatest benefit of patients and the organization.

Helps patients and families make difficult decisions in order to move discharge plan forward.

Provides supervision as required to social work staff.

Intervenes to decrease emotional crisis situations.

Assesses, creates, implements, evaluates, and modifies timely discharge plans.

Subscribes to and upholds established standards of behavior.

Works collaboratively and supports efforts of other team members.

Supports quality improvement initiatives through team participation, data collection, process change implementations, and other activities.

Maintains confidentiality and protects sensitive data at all times, including patient information, proprietary information and personnel information.

Adheres to organizational and department-specific safety standards and guidelines.

Performs related duties as assigned.

MINIMUM QUALIFICATIONS:

EDUCATION:  Master’s in Social Work or equivalent.

EXPERIENCE: Five (5) year of related social work experience.

LICENSURE OR CERTIFICATION: HeartSaver First Aid and CPR Certification. Must have or be able to obtain Arizona State licensure as a LCSW at the time of hire.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of crisis intervention skills and techniques, psychosocial factors influencing behavior, counseling concepts, and discharge planning.

  • Skill in evaluating cases and establishing discharge plans.

  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.

  • Ability to complete routine reports and correspondence.

  • Ability to listen and accurately interpret others' communication or instructions to take appropriate action.

  • Ability to speak effectively before groups of customers or employees of organization.

  • Ability to provide therapeutic techniques to provide problem resolution.

  • Ability to define problems, collect data, establish facts, and draw valid conclusions.

  • Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
