SUMMARY:

The goal of the RN Coordinator is to promote seamless care from referral to discharge. The RN Coordinator works in direct collaboration with the cardiologists, cardiac surgeons, nurse practitioners, nurses, and other team members; participates in the ongoing assessment of coronary artery bypass graft (CABG), valve , and structural heart pre and post op patients.  The Nurse Coordinator participates in clinical decisions regarding the management of in-patients and out-patients; provides and promotes patient education; provides, promotes and encourages nursing staff education and involvement; and serves as a liaison between physicians, patients, families, nursing staff and referring facilities to promote quality and continuity of patient care.

ESSENTIAL FUNCTIONS:

Records patient’s health information and establishes/records care.

Participates in Pre-, Peri- and Post-Procedure activities directly involving interactions with the patient and/or family; can include but is not limited to treatment plan review, education, testing coordination, rounding, medical record review, status reports, follow-up phone calls, and other activities determined to be relevant.

Consults and collaborates with members of the health care team in the assessment and management of health care problems; functions as a liaison between physicians, nursing, technologists and area support staff.

Assists in the development, implementation, and revision of nursing protocols and standards of care. Develops patient education programs and tools.

Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care plans in accordance with nationally recognized care guidelines.

Researches and provides advice on patient care processes, nursing processes, and larger-scale quality improvement projects.

Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, and social services staff).

Adheres to and supports team members in exhibiting TMCH values of integrity, community, compassion, and dedication.

Performs related duties as assigned.

 

MINIMUM QUALIFICATIONS

EDUCATION: Bachelor’s degree in Nursing from a qualified, nationally-accredited nursing program required.  Master’s degree preferred.

EXPERIENCE:  A minimum of three (3) years of related experience require, preferably in an acute care setting with experience as a health/patient educator or patient advocate, preferred area of clinical experience in the program area for the position.

LICENSURE OR CERTIFICATION:  Current RN licensure permitting work in the State of Arizona and Basic Life Support (BLS) required.

KNOWLEDGE, SKILLS AND ABILITIES

  • Knowledge of nursing care and assessment best practices as well as related clinical components.

  • Ability to be sympathetic and emotionally supportive to the patient/family throughout the care continuum

  • Highly organized and able to multi-task.

  • Must maintain positive attitude through often stress conditions.

  • Knowledge of regulations and policies as it relates to patient evaluation, care and information.

  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

  • Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.

  • Ability to write reports, correspondence, and procedure protocols.

  • Ability to effectively present information and respond to inquiries or complaints from patients and/or their representatives, and the general public.

  • Ability to work with concepts such as fractions, percentages, ratios, and proportions, and to apply mathematical operations to solve or analyze job-related situations.

  • Ability to identify positive or negative variances from expected outcomes.

  • Demonstrated compassion, integrity and ability to work with a wide range of disciplines

  • Requires problem solving, decision-making, critical thinking, analytical, and deduction skills.

  • Preferred knowledge of integrated medicine modalities

  • Requires excellent leadership, organizational, written, verbal communication, and interpersonal skills.

  • Must be able to work in a self-directed environment, with an ability to work with teams

  • Excellent presentation skills; Ability to implement professional and community based education programs.

  • Above-average skill in the use of Microsoft Office products, including Excel, Word, PowerPoint, etc required; familiarity with spreadsheets and statistical data is necessary (as will be related to quality improvement and tracking projects).
