SUMMARY:

Provide project oversight and management for a variety of strategic operations related to the Medical Neighborhood Program to address social determinants of health for the community, including Tucson Community Collaborative (TC-3), community health initiatives and population health strategies. Leads projects, oversight of Medical Neighborhood staff, prepares status reports, performs practice valuation assessments, develops and maintains project schedules and budgets.

Develops, builds, plans and executes projects that promote the strategic goals of the TMC Foundation and the Medical Neighborhood project.

 

ESSENTIAL FUNCTIONS:

Provides project management and oversight and strategic direction related to the Medical Neighborhood Program in accordance with the mission and goals of the organization.

Develops and manages budget and operating plan for the program(s).

Develops an evaluation method to assess program strengths and identify areas for improvement.

Develops policy, protocols and staffing model for the activities of the Medical Neighborhood Program.  

Assist with grants and funding proposals to guarantee uninterrupted delivery of services the program

Manage a team with a diverse array of talents and responsibilities.

Ensues goals are met in areas including customer satisfaction, safety, quality and team member performance.

Implement and manage changes and interventions to ensure project goals are achieved.

Communicate with stakeholders regarding the Medical Neighborhood Program, project issues and decisions on services.

Produce accurate and timely reporting of program status throughout its life cycle.

Coordinate and manage events and workshops.

Reporting on program performance to executive team and directors.

Gather feedback and present insights and report on quality performance metrics as determined by key stakeholders.

Identify opportunities for continual improvement and analyze program risks.

Develop industry partnerships.

Performs related duties as assigned. 

 
MINIMUM QUALIFICATIONS

EDUCATION: Graduated from a qualified, nationally-accredited nursing program required. Bachelor’s degree preferred.

EXPERIENCE:  Five (5) years of related experience, preferably in an ambulatory care setting.

LICENSURE OR CERTIFICATION:  Arizona State RN Licensure
