Job Title: Wound Care Registered Nurse                              Department: Nursing

 

Reports To:  Director of Nursing

 

 

POSITION SUMMARY

The Wound registered nurse is responsible for providing direct and indirect patient care in inpatient outpatient areas. He/she is to provide education, consultation and assistance to the physicians, and nursing staff to reach and maintain the goal of zero facility acquired pressure ulcers and exceptional wound healing. The Wound Care nurse accomplishes these goals through teaching, training and delivery of skin risk assessments, implementation of prevention interventions, accurate skin care, administering wound treatments prescribed by the attending physician, and documentation, consistent with accepted standards of care and within the scope of RN/CNA training and licensure.  This position will be assigned to perform and assist with assessments, interventions, and evaluations for patients with actual or potential wound/ostomy problems. Activities include, but are not limited to, dressing changes for patients with, implementing plans of care created by Wound RN, assisting in management of pressure ulcers and other wound/skin problems.

 

ESSENTIAL FUNCTIONS

  1. Provide excellent nursing care to patients suffering from wounds.

  2. Assess and evaluate patients with wounds and injuries.

  3. Obtain cultures to assess wounds and injuries.

  4. Evaluate wounds and injuries for infections or other illness factors that causes wounds.

  5. Initiate nursing care procedures in managing acute and traumatic wounds.

  6. Coordinate with rehab, traumatic and nutritional nurses in nursing patients with wounds.

  7. Ensure optimum patient care delivery in wound care nursing procedures.

  8. Educate and counsel patients and their families on wound care processes and issues.

  9. Demonstrate wound care procedures to other care givers.

  10. Sanitize and maintain the premises of wound care neat, clean and hygienic.

  11. Identifies initial and ongoing patient/family/significant other’s education as an integral part of care and implements appropriate measures to meet these as evidenced by documentation.

  12. Initiates discharge planning based upon admission assessment, makes appropriate referrals, and monitors patient/family/significant other’s process and readiness for discharge, collaborating with health team members as appropriate. Performs assessments focusing on the physical and psycho-social health status of the patient in an ongoing, timely, and systematic manner as evidenced by documentation.Participates in formulating the outcomes based on the plan of care which is prioritized by nursing.

  13. Performs concise, accurate, and legible documentation according to standards designated for pertinent circumstances as evidenced by documentation.

  14. Provides a safe, clean, comfortable environment for patient as evidenced by observation.Identifies and reports potential and active environmental safety hazards.

  15. Direct or supervise less-skilled nursing or healthcare personnel.

  16. Attends at least 75% of staff meetings; remains accountable for content of missed staff meetings.

  17. Displays flexibility in scheduling to meet staffing needs of the hospital.

  18. Provides nursing care that meets the physical, spiritual, and social needs of patients and their families.

  19. Attends all mandatory in-services.

  20. Adheres to all Infection Control policies and practices, including hand hygiene, personal protective equipment, isolation precautions, and healthcare worker immunizations.

  21. Strives to gain additional knowledge and skills, including certifications.

  22. Administers medication accurately and per physician’s order.

  23. Knowledgeable in the use of specialized equipment, treatment procedures, policies, practices, and standards of clinical area.

  24. Maintains harmonious relationship with other departments and co-workers, providing support and assistance to co-workers across units to ensure quality patient care.

  25. Maintains patient confidentiality and does not discuss patient in public areas such as cafeteria, hallways, parking lots or loudly so others can hear.

  26. Demonstrates ability to lift, transfer and transport patients in wheelchair, bed or stretcher utilizing appropriate transfer device.

  27. Reduces the risks of accidental injury or exposure by following established safety guidelines.

EDUCATION AND EXPERIENCE REQUIREMENTS

  1. Active Arizona RN License

  2. ADN-Minimum Education

  3. BSN - Preferred Education

  4. Current specialty certification in wound care.

  5. 3 year of wound care experience preferred.

  6. Current AHA BLS, ACLS and PALS Certification required

     

     

    SKILLS

    Answer Telephones, Computer Data Input, Legible Documentation, Critical Thinking, Clinical Skills, Written and Verbal Communication

     

    Machines: Diagnostic Equipment, EKG, Cardiac Monitors, Glucometer, Kangaroo Pump, Facsimile and Copier, Agilia IV pumps, IMED,                       Zoll Defibrillator
