Registered Nurse (RN) Nurse Coordinator - Palliative Care
A professional clinician with demonstrated ability to provide relationship- based care within a collaborative multi-disciplinary team. Requires strong organizational skills with capacity to easily adapt to daily challenges and prioritize appropriately. Promotes and provides patient / nursing education focused on advanced care planning.
Coordinates multidisciplinary planning conferences and helps patients/families process and synthesize information delivered by providers.
In person triage of consults and notification to providers of cases requiring immediate intervention.
Track and trend triage results to assist in defining/assigning generalist vs. specialist palliative care consults.
Develops standardized templates for efficient and consistent documentation by all team members.
Maintains, tracks, monitors and reports on benchmarks developed for the program; demonstrating outcomes and recommendations for performance improvement activities.
Serves as liaison for all needed EPIC changes, involving appropriate team members as needed and ensures education roll out in a timely manner.
Develop and maintain resources to provide culturally diverse patient education.
Assists in the development, implementation, and revision of protocols and standard work related to palliative care and advanced care planning for inpatient units, ED and the Tucson community.
Maintains compliance with appropriate regulatory, federal, and state mandated requirements.
Demonstrates flexibility with work flow and daily schedule to manage changing provider and patient priorities. .

Performs related duties as assigned.
EDUCATION: Bachelor’s degree in Nursing required. Master’s degree preferred.
EXPERIENCE: A minimum of three (3) years of related experience required, preferably in an acute care hospice and/or palliative setting with experience as a health/patient educator or patient advocate.
LICENSURE OR CERTIFICATION: Current RN licensure permitting work in the State of Arizona and Basic Life Support (BLS) required.
KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of nursing care and assessment best practices.
Knowledge the clinical components related to the program assigned.
Knowledge of integrated medicine modalities.
Skill in providing analytical and deduction analyses.
Skill with a variety of computer applications such as: Microsoft Office competency required; including spreadsheets and statistical data (as well as programs related to quality improvement and tracking projects).
Ability to write reports, correspondence, and procedure protocols.
Ability to effectively present information and respond to inquiries or complaints from patients and/or their representatives, and the general public.
Ability to identify positive or negative variances from expected outcomes.
Ability to implement professional and community based education programs.
Ability to be sympathetic and provide emotional support to the patient and family throughout the care continuum
Ability to maintain positive attitude through often stress conditions.
Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations
