Patient Relations Advocate Per Diem
Serves as the conduit for goal concordant care issues and situations when a patient is transferred to a long-term care community. Provides guidance and instruction to patients, families and facility staff as needed to obtain patient’s healthcare directives. Responsible for uploading healthcare directive documents into the electronic medical record,
- Facilitates communication between patients, families and facilities, and as appropriate payer associated, long term care managers to ensure care wishes are known.
- Responds to unusual or non-routine inquiries to ensure patient and/or family understanding of healthcare directives, and treatment plans.
- Encourages understanding and adherence by both staff and patients to TMCH philosophy on patient’s goals of care.
- Interacts with End Of Life Care Partnership, and participates in funding reporting process and attendance at EOLCP meetings as appropriate
EDUCATION: Bachelor’s degree or an equivalent combination of relevant education and experience.
EXPERIENCE: Three (3) - five (5) years’ experience, preferably in an acute care setting with a background in public relations in a patient advocacy capacity in an interdisciplinary patient care setting.
