Nurse Practitioner - Palliative Care-Outpatient
This role is designed for our new outpatient Palliative Care program in order to help our patients outside of the hospital. The process would start with the inpatient palliative care team requesting a consult and this NP would pick up the process from there. Ideally, this person would work Monday-Friday 8am-5pm, no weekends or holidays as the program is just coming to fruition.
An exempt position responsible for assessing, managing, prescribing and evaluating appropriate therapeutic and pharmacologic interventions, providing palliative care consultation to seriously ill patients and their families. As a member of the palliative care multidisciplinary team, the nurse practitioner plays a pivotal role, working closely with physicians and other clinicians.
Conduct comprehensive, focused assessments of patients and families with palliative care needs.
Perform bedside palliative care patient/family evaluation
Assist with diagnosis, treatment, and management of acute and chronic health conditions.
Formulate, implement, monitor, evaluate and modify palliative care plans with input from palliative care team and other collaborating care providers.
Work with patients and families to determine their understanding of the patient's condition, proposed treatment plan and treatment options.
Assist in all facets of care coordination for palliative care referrals.
Engage in quality improvement in patient care services and department functions, including evaluating the effectiveness of the health care delivery system in meeting the health care needs of palliative care patients and families.
Advocate for improvements in palliative care through participation in professional, private, and governmental groups.
Provide clinical expertise and guidance for undergraduate and/or graduate nursing students in the specialty area.
Act as resource person for clinical information and education as requested by community agencies.
Assess the environment for potential need for consultation.
Plan clinical and educational interventions which are consistent with individual/organizational needs and goals, and evaluate change/outcomes.
Implement patient and family education in collaboration with the interdisciplinary team.
Participate in palliative care research, including quality and performance improvement initiatives.
Define goals for professional growth, and participate actively in professional activities and organizations.
Demonstrate accountability for evaluation of clinical practice, including adherence to organizational, state and national practice standards and codes.
Consult with palliative care physician or designees as needed, communicate with primary physician and relevant consultants regarding services provided.
Provide training and continuing education for staff
Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
Provide IT skills and familiarity, by using the basic functionality of the computer, Word, Excel, Outlook e-mail and attachment capabilities.
Maintain compliance with regulatory requirements.
Perform other evaluative and supportive duties, due to the advanced nature of training and the Licensed Independent Practitioner (LIP) status, as needed.
Research and provide advice on patient care processes, nursing processes, and larger-scale quality improvement projects.

EDUCATION: MSN required. ACNP is preferred but FNP’s and CNS’s may apply. Advanced Hospice and Palliative Care Certification is preferred.
EXPERIENCE: Minimum three years of professional nursing experience in a clinical setting, including care of patients with complex, chronic and/or life-threatening conditions, and those at end of life is required. Clinical experience in oncology, hospice or intensive care is preferred.
LICENSURE OR CERTIFICATION: National certification as a Nurse Practitioner or must be National Certification as a Nurse Practitioner eligible, must become credentialed at TMC through the Professional Staffing Office prior to appointment. Must have and maintain a current Arizona state license as a Registered Nurse and Nurse Practitioner including prescriptive authority. Must obtain and maintain a current Federal Drug Enforcement Agency License for prescribing controlled substances. Must obtain and maintain current certification in Basic and Advanced Cardiac Life Support; for Mental Health only Crisis Prevention Instruction (CPI) required.
KNOWLEDGE, SKILLS AND ABILITIES:
The Palliative Care APRN is an advanced practice nurse, clinically expert in patient care, who can provide a high standard of care to patients needing palliative care following an established protocol with a physician(s).
He/she provides direct patient care in collaboration with physicians and other members of the Interdisciplinary team: focusing on patients with complex illness and those who need pain and/or symptom management.
The APRN performs patient assessments including a medical history and physical exam and in collaboration with the physician develops an appropriate plan of care.
This includes but is not limited to medical diagnosis, symptom management, prescription, and teaching.
The APRN manages complex patient problems, patient and family communication, and care planning needs.
He/she combines clinical practice with education, research, consultation and leadership.
The APRN works directly with staff, administration, patients and families to ensure quality care across the continuum as an expert in palliative care.
He/she integrates current knowledge of professional issues, trends in health care and technological advances into scope of practice.
Additionally, the APRN participates in the collection of research data, quality improvement, discharge planning and coordination of patient care for palliative care patients.
Shares in the supervision of RNs, LPNs, medical assistants, social workers, and others depending on departmental organization.
