This role is a combination of inpatient and outpatient hospice. The ideal schedule for this role is Thursday-Monday with Tuesday/Wednesdays as off days. (this may be flexible)
As a Nurse Practitioner (NP) for TMC Hospice, you will work autonomously to provide advanced practice nursing services through palliation and management of the terminal illness for patients receiving inpatient services, respite services, and home hospice services. As part of the multidisciplinary team, you will work collaboratively to develop the plan of care for our inpatients and home care patients who reside in their own private residence, a skilled nursing facility, adult care home or other living arrangements.
- Communicate significant findings, changes in condition or environment, or other concerns to members of the interdisciplinary team.
- Complete face to face encounters to assess disease related changes, decline in function, and effectiveness of pain and symptom management regimen
- Participate in weekly interdisciplinary group (IDG) conferences
- Participate in rotating on-call schedule
EDUCATION: Master’s degree in Nursing or related area, or an equivalent combination of relevant education and experience.
EXPERIENCE: Minimum of one (1) year experience as a Nurse Practitioner or minimum of one (1) year experience as a hospice Registered Nurse
LICENSURE OR CERTIFICATION: National certification as a Nurse Practitioner or must be National Certification as a Nurse Practitioner eligible, must become credentialed at TMC through the Professional Staffing Office prior to appointment. Must have and maintain a current Arizona state license as a Registered Nurse and Nurse Practitioner including prescriptive authority. Must obtain and maintain a current Federal Drug Enforcement Agency License for prescribing controlled substances. Must obtain and maintain current certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS); for Mental Health only Crisis Prevention Instruction (CPI) required.
