Responsible for the daily over site of TMC hospice outreach programs including but not limited to TMC based Hospice outreach and select community Hospice outreach programs. Utilizes strong communication skills to engage staff, healthcare providers and a diverse community in various programs and Hospice opportunities. Serves as TMC Hospice liaison to community-based coalitions.
Performs all related duties as coalition coordinator for TMC based Hospice outreach and outreach programs.
Assists with Hospice program development.
Serves as liaison to community agencies, physician offices, and Hospice focused coalitions.
Develops and maintains strong relationships with internal and external stakeholders for Hospice marketing and community affairs purposes.
Plans layout of printed pieces, arranging sequence of topics and Hospice outreach documentation; directs the preparation and publication of product information brochures and similar materials.
Works with Hospice leadership to facilitate market growth; identifies any issues with communications channels and streamlines as needed.
Serves as contact with outside agencies on matters relating to TMC Hospice; develops and maintains community relationships for marketing/community networking purposes.
Exhibits strong initiative and self-motivation; works well independently and identifies and takes action to resolve barriers to quality, workflow and customer satisfaction.
May coordinate or assist in the coordination of TMC Hospice recognition events.
EDUCATION: High School Diploma or equivalent; preferred Bachelor’s degree.
EXPERIENCE: One (1) year of experience with Hospice and/or Community Outreach.
