SUMMARY:
Provides oversight, coordination, training and education, and strategic development for the Geriatrics program. Oversees data development and tracking, certification requirements and monitors team responses for critical patient care. Maintains Database and runs abstracts to evaluate performance for quality control and recommends improvements as required. Manages geriatric inpatient programs, community outreach/program development and coordinates the geriatric clinic.
ESSENTIAL FUNCTIONS:
Coordinates the interdisciplinary Geriatric team.
Assists and recommends protocols and guidelines for best practice treatment of geriatric patients across the continuum of care.
Manages geriatric education for patient and staff populations. Designs, develops and delivers course materials as applicable. Provides direction, instruction and guidance for all staff involved in geriatric patient care.
Oversees the tracking process for data related to throughput of the geriatric patients and provides data to the Director and Medical Director to initiate changes as appropriate and to improve outcomes.
Assists in the development of strategic plans for the growth of Geriatric Services.
Creates and implements community outreach programs and grows community network for geriatric services.
Provides, manages and coordinates care; applies analytical reasoning, rational problem-solving skills, using evidence-based information and clinical judgment; acts as clinical resource to other staff members
Consults and communicates with physicians to ensure proper care and follow up of patients.
Ensures accurate data collection, coordinates documentation efforts of staff and physicians, and educates the teams on the importance of that documentation.
Evaluates outcomes of geriatric patient care, consults with other specialists as needed and adjusts nursing care processes as necessary to ensure optimal patient care.

MINIMUM QUALIFICATIONS:
EDUCATION: Bachelor’s degree in Nursing, Master’s degree is preferred; or an equivalent combination of relevant education and experience with a Master’s degree in Social Work, Occupational Therapy or Physical Therapy.
EXPERIENCE: Five (5) years of experience in program coordination in a healthcare setting. One (1) year of experience in adult acute care; geriatric experience preferred.
LICENSURE OR CERTIFICATION: For Nursing degree, current RN licensure permitting work in the state of Arizona. For Social Work degree, Licensed Certified Social Worker (LCSW) required. For all, current Cardio Pulmonary Resuscitation (CPR) and Advanced Cardiac Life Support (ACLS) required.
KNOWLEDGE, SKILLS AND ABILITIES:
Demonstrated communication and presentation skills. Ability to speak and communicate effectively.
Collaborative working style and ability to work with diverse populations
Knowledge of chart abstraction, education and marketing for geriatric programs preferred.
Skill in assessing performance and recommending improvements.
Skill in developing training materials and educating staff in the proper responses for geriatric care issues.
Skill in the use of computer systems and programs, such as MS Word, Excel, e-mail and attachment capabilities, Outlook, and PowerPoint.
Skill in communicating with staff involved in geriatric care and physicians to ensure the proper care of patients.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
Ability to listen and accurately interpret others’ communication or instructions to take appropriate action.
Ability to define problems, collect data, establish facts, and draw valid conclusions.
Ability to interpret an extensive variety of instructions and deal with several abstract and concrete variables.
