SUMMARY:

Plans, develops, implements, documents, and evaluates diabetes education for inpatients, outpatients and their families.  Participates in community education programs.

 

ESSENTIAL FUNCTIONS:

Plans curriculum content and sequence and prepares schedules for classes and other activities.

Coordinates classroom/meeting room facilities and instructional aids; researches, selects and develops course materials.

Communicates and interfaces with hospital personnel, patients, medical staff and family members to ensure high-quality patient care for patients with diabetics.

Documents patient care efficiently, timely and effectively through written communication.

Demonstrates procedures and testing materials; encourages patients to participate in procedures and techniques, including instructing patients in giving themselves injections.

Develops treatment plan and discuss proposed course of treatment with patient’s attending physician.

Prepares, organizes, directs, conducts and supervises programs for orientation/training and in-service education; f ormulates and develops plans, procedures and programs to meet specific training needs and problems.

Ensures educational needs of staff are met; acts as an education resource to nursing community and health care team; develops referral sources and community education programs.

Investigates new products, equipment and techniques and makes recommendations to nursing director.  Participates in demonstration of these to department staff.

Implements and maintains established department, unit and hospital policies, procedures and objectives, and environmental and infection control policies.

Adheres to TMC organizational and department-specific safety, confidentiality, values, policies and standards. 

Performs related duties as assigned.

 

MINIMUM QUALIFICATIONS

EDUCATION:   Nursing degree, MSW, or a Bachelor’s degree in Nutrition or related field.

EXPERIENCE:   Two (2) years of diabetes care or education experience.

LICENSURE OR CERTIFICATION:  C urrent RN and/or RD licensure permitting work in the state of Arizona, and/or LMSW and Basic life support (BLS) certification required.  Certified Diabetes Educator (or ability to obtain CDE certification within a year of hire) preferred .

KNOWLEDGE, SKILLS AND ABILITIES: 

·         Knowledge of diabetes mellitus and its treatment and management. 

·         Knowledge of pediatric and adult education principles.

·         Skill in presenting educational material to a variety of age groups.

·         Skill in developing training materials and presentations using a variety of media to engage participants.

·         Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.

·         Ability to write reports, business correspondence, and procedure manuals.

·         Ability to effectively present information and respond to inquiries or complaints from employees, patients and/or their representatives, and the general public.

·         Ability to calculate figures and compute rate, ratio, and percent and to draw and interpret bar graphs; ability to apply basic algebraic concepts.

·         Ability to define problems, collect data, establish facts, and draw valid conclusions.

·         Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
