Functions as a Clinical Care Coordinator (CCC) in a team and mission-based setting to provide for the oversight of Case Management, Utilization Review and Infection Preventionist for Northern Cochise Community Hospital (NCCH) / Rural Health Clinics (RHCs). CCC will coordinate health care and other related services for Acute, Observation, Skilled, Swing, Hospice, and Emergency Department (ED) patients through multidisciplinary care team, other facility case managers and providers. CCC assists with admission and discharge planning through multidisciplinary team meetings. CCC works with Insurance companies and case managers to ensure clinicals are sent as required and authorizations for admissions are obtained and documented in the patients record.

CCC will assist in the promotion and application of infection control measures to prevent and/or control infections among patients, and employees at the hospital and rural health clinics. CCC designated to provide social services as needed. CCC enforces and complies with Center for Medicare & Medicaid Services (CMS) and Arizona Department of Health Services (ADHS) regulatory requirements. Multitasking, self-motivation, organizational skills, and the ability to work in a fast-paced environment are required skills and traits. The CCC must actively participate in NCCH performance improvement and continuous quality improvement activities. Perform any other duties needed to help drive our Vision, fulfill our Mission, and abide by our Organization’s Values. RN Designation Required.

NOTE: Exempt Position requires collaborative RN rotation for weekend call and shift coverage for increased census/acuity or call-offs.

Supervision Received: Works under the direct supervision of the Chief Nursing Officer (CNO) or designated Administrator. Exercises progressive levels of autonomy in day-to-day activities based on satisfactory job performance.

Special Job Conditions:  Will carry on call phone for questions from staff regrading patients' needs. May be subject to call back during emergency conditions.

EDUCATION AND EXPERIENCE:

REQUIRED: Associate Degree in Nursing required.  Minimum of three years’ experience as an acute care registered nurse.

PREFERRED: Bachelor of Science in Nursing (BSN). Experience in areas of direct patient care on the Medical Floor, Skilled Nursing Facility (SNF) and/or Emergency Department (ED), highly desirable. Case Management, Utilization Review, and Infection Control experience preferred.

Working experience with programming of the electronic medical records (EMR), strongly desirable.

Working knowledge of the various insurance carriers, Medicare, AHCCCS, and other third-party payors and their criteria for approved medical coverage and reimbursements for medical care provided, strongly desirable.

CERTIFICATES, LICENSES, REGISTRATIONS:

REQUIRED: Current, unrestricted Arizona Registered Nurse license. BLS and ACLS upon hire; PALS within 30 days of hire.

Necessary Knowledge, Skills, and Abilities:

  • Proficient knowledge of Nurse Practice Act, the nursing process and standards of care.
  • Ability to work with various insurance carriers, Medicare, AHCCCS, and other third-party payors during case management process to obtaining approval for patient hospitalization and/or extension of patient hospitalization by providing proof of medical necessity, etc. 
  • Ability to understand and follow guidelines of Milliman admission criteria and how to apply them to patient admissions and/or ongoing patient hospitalization.
  • Knowledge of and ability to use internal electronic medical records system for Case Management and Utilization review purposes.
  • Ability to prioritize, organize tasks, maintains patient care documentation, and manage data for utilization review purposes.
  • Excellent verbal and written skills required as well as clinical decision-making skills.
  • Ability to work with providers, patients and families of patients as part of Case Management.
  • Knowledge of HIPAA privacy requirements and medical terminology.
  • Establish and maintain solid working relationships through communication, cooperation, and positive interactions with all patients/clients, employees, staff, providers, and vendors.
  • Willingness to work a flexible schedule.
  • Demonstrated ability to self-motivate and work independently with limited supervision.
  • Strong customer service skills along with excellent written and verbal communication skills.
  • Positive communication skills; ability to successfully interact with all customers; continuous demonstration of courteous, cooperative and service orientated behavior.
  • Supports and maintains a culture of safety, quality and positive patient experience.
  • Basic computer skills, and ability to use office equipment to include but not limited to copy machine, faxes, EMR, etc.,
