SUMMARY:

A non-exempt position directly responsible for day-to-day supervision of the Chronic Neurological Disease Patient.  Provides leadership that promotes excellent customer service and professional conduct.  Upholds clinical standards and compliance with state and federal governing agencies.

 

ESSENTIAL FUNCTIONS:

Works with Manager developing and monitoring patients and programs in the Chronic Neurological Disease Department.

Ensures that qualified patients are enrolled in the program.

Collaborates with leaders and physicians to ensure the organization meets or exceeds patient experience expectations.

Provides non-medical support to patients enrolled in the program and their families.

Develops relationships with enrolled patients, their healthcare teams, and local community resources and services.

Identifies patient needs and directs them to sources of emotional, financial, administrative, and cultural support.

Meets with patients (and families) in the hospital with a chronic neurological diagnosis, such as dementia, Alzheimer’s disease, and Multiple Sclerosis.

Communicates with patients and their families to address non-physical needs.

Monitors and evaluates quality assurance activities, ensuring compliance with standards, laws, regulations, and procedures.

Resource for providers, medical assistants, and research team.

Develops support groups for patients and families.

Plans and executes patient education programs.

Participates in quality improvement process.

Respects / promotes patient rights and responds appropriately to emergencies.

 

MINIMUM QUALIFICATIONS

EDUCATION:  Master’s degree in Social Work or a related field.

EXPERIENCE:  Two (2) years of social work experience; preferably in an acute care setting.

LICENSURE OR CERTIFICATION:  Arizona State Licensure as a LCSW preferred.
